Job summary
Skilled Healthcare of Staten Island is seeking an attentive Registered Nurse (RN) to perform comprehensive clinical assessments and develop individualized care plans using the UAS-NY framework. You will evaluate participants, coordinate care, and ensure compliance with state standards while promoting overall well-being.
Responsibilities include conducting UAS-NY assessments, reviewing factors for care levels, interpreting data, and maintaining accurate documentation to support high-quality care
Qualifications
- Active NY RN license
- Current UAS-NY Certification
- Experience with UAS-NY
- MLTC/Managed Medicaid experience a plus
- Strong organizational and communication skills
- Proficient with electronic documentation systems
Responsibilities
- Conduct comprehensive face-to-face clinical assessments for participants per UAS-NY guidelines and NYS regulations.
- Complete UAS-NY evaluations to determine eligibility and continued participation.
- Assess prospective participants by reviewing clinical, psychosocial, and environmental factors for care level decisions.
- Collect and interpret clinical data to support evidence-based care decisions.
- Develop and update individualized service plans tailored to participant needs.
- Communicate with participants, families, providers, and team members for coordinated care.
- Demonstrate understanding of complex chronic bio-psychosocial needs.
- Maintain compliance with regulatory, documentation, and organizational standards.
