Job summary
Senior Community Care of Kentucky PACE in Louisville, KY seeks a skilled Nurse Practitioner to provide primary care for elderly participants in a PACE program. You will perform physical assessments, collaborate with an interdisciplinary team, and develop comprehensive plans of care.
The role offers a Monday–Friday schedule, with the opportunity for prescriptive authority within one year and a sign-on bonus of $5,000. A strong geriatric care focus and compassionate approach are essential.
Qualifications
- Graduate of a school of professional nursing and RN licensed in the state of employment.
- Certified nurse practitioner with prescriptive authority to practice.
- GNP certification preferred; geriatric care experience desirable.
Responsibilities
- Provide primary care to PACE enrollees under supervision and Medical Director oversight.
- Perform in-person physical assessments and semi-annual reassessments; develop and implement plans of care.
- Coordinate care with the Interdisciplinary Team and Primary Care Physicians; discharge planning as needed.
