Job summary

EverHome Healthcare is seeking a Per Diem RN to conduct home health visits in King and Snohomish counties. The role involves nursing assessments, care management, and delegations with a focus on compassionate patient service.

Responsibilities include writing Plans of Care, supervising caregivers, and on-call coverage. WA RN license and vehicle are required; experience in rehab or home health is preferred.

Qualifications

• Must be a WA-licensed RN and graduate of an accredited nursing program.
• Ability to practice independently in delivering nursing care.
• 2–3 years of clinical experience in rehab, home health, hospital, or hospice.
• English reading/writing proficiency.
• Valid WA driver's license and reliable vehicle.

Responsibilities

• Perform RN home health visits and assessments.
• Develop and write Plans of Care and ensure adherence.
• Delegate tasks to caregivers and provide training as needed.
• Monitor client condition and report changes to Clinical Director.
• Complete clinical documentation accurately.
• Rotate on-call for urgent nursing issues.

Weprovidea wide range ofservices to help individualsremainin their homes.Someof our current client careincludesworking with clients with dementia,spinal cordinjuries,brain injury,andboneandjoint injury.Some of the nursing visits can include but are not limited toinfusion therapy,wound care, setting medications, providing injections, compression wraps, urostomy appliance change,and ventilator care.We alsoprovideCare Management servicesas well as RN Delegation. Preferred candidates will have a wide range of experience in many areas of nursing including supervising caregivers.

Duties, Responsibilities, & Accountabilities Including but Not Limited To:

• RNHome Healthvisits
• Responsible for RN delegations and documentation.If youdon’thave experience with this, we can train you
• WritePlansof Care’s and assures they are being followed
• Observes client for changes in condition and reports such changes to Clinical Director

• Documents nursing observations, interventions and evaluations of the care beingprovided
• Doing assessments which includes opening new client cases
• Address emergent nursing issues
• Supervise Caregivers (CNA, HCA) byprovidingcoaching and training
• Clinical administrative work

• Rotate being on-call for urgent nursing matters via on phone or visits

Qualifications – Knowledge Required:

• Must be an RN in the State of Washington and graduated froman accredited School of Nursing
• Mustdemonstratean ability to practice independentlyin the delivery of nursing care
• 2 to 3 years’ clinical experience; preferably in rehabilitation, home care, hospital, or hospice
• Ability to read and write in English
• Require reliable vehicle for use when traveling to worksites
• A valid Washington State Driver’s License

• Proof of current auto liability insurance
• Computer literate with experiencein different computer programs
• Strong grasp of legal and ethical issuespertaining toconfidentiality
• Creative, flexible, and innovative
