Job summary

Nazareth Hospital, part of the Trinity Health system, is seeking a Registered Nurse for the Float Team in Nazareth Hospital, Philadelphia, PA. This full-time position covers 12-hour days (7:00 am–7:30 pm) with weekend rotations and holiday requirements.

The RN will assess, plan, implement and evaluate patient care, coordinate with the team, and participate in discharge planning and rounds to ensure high-quality care.

Qualifications

• BSN or BS/BA in Nursing required.
• Current RN Pennsylvania state license required.
• Current BLS required. ACLS required or obtained by orientation if applicable to area of practice.
• Previous acute care experience preferred.

Responsibilities

• Provides total, comprehensive care to patients by applying the nursing process in a structured, policy-driven manner.
• Communicates proactively with the health care team about plan of care, patient changes, progress, discharge plan and other information.
• Performs accurate and thorough nursing assessments and documentation.
• Reassesses patient condition per policy and integrates clinical data into the care plan.
• Participates in discharge planning and multidisciplinary rounds; supports patient and family grieving when appropriate.
