Job summary

The Nazareth Hospital in Philadelphia is seeking a Registered Nurse to join the Float Team. You will assess, plan, implement and evaluate patient care across units, coordinating with the health care team to ensure safe, high-quality outcomes.

This full-time 12-hour dayshift role requires a BSN or higher, PA RN license, BLS, ACLS, and prior acute care experience. You will participate in discharge planning and contribute to a collaborative, healing environment with commitment to patient-centered

Qualifications

• BSN or higher nursing degree required.
• Current RN Pennsylvania state license required.
• BLS required; ACLS required or obtained on orientation.
• Previous acute care experience preferred.

Responsibilities

• Provide total, comprehensive nursing care using the nursing process.
• Communicate care plans, changes in condition, and discharge plans.
• Complete admissions and assess patients with a multi-system approach.
• Document assessments, interventions and outcomes per policy.
• Reassess patient condition and apply clinical data to care plans.
• Participate in discharge planning and multidisciplinary rounds.
• Assist with grieving process and end-of-life care as appropriate.

Shift: 12 hour dayshift (7:00 am - 7:30 pm) - weekend rotations and holiday requirements

Location: Nazareth Hospital - Philadelphia, PA

Position Purpose

The Registered Nurse (RN) is responsible for assessing, planning, implementing and evaluating the delivery of patient care. The professional nurse assumes the responsibility and accountability for the delegation of patient care to other members of the healthcare team.

As a Registered Nurse, You Will

• Provides total, comprehensive care to patients by applying the nursing process in an organized and systemic manner according to hospital policy.
• Communicates proactively to the health care team, plan of care, changes in patient condition, the patient progress, the discharge plan and other relevant information.
• Completes admission according to policy utilizing a multi-system approach for the assessment and reassessment.
• Documents assessment/reassessment, plan, interventions and outcomes according to policy and interprets and utilizes clinical data in the plan of care:
• Performs accurate and thorough nursing physical assessment and documents thoroughly.
• Reassesses patient condition per policy.
• Interprets & utilizes clinical data in the plan of care.
• Assesses patient for potential risk factors.
• Recognizes and integrates patient special needs and individualizes care.
• Includes pain management and supportive care issues in plan of care.
• Adheres to infection control standards specific to patient care.
• Assumes accountability for managing the delivery of patient care.
• Responsible for learning policies and nursing procedure and communicates to Performs accurate and thorough nursing physical assessment.
• Participates in discharge planning, using a multi-disciplinary approach. Participates in multidisciplinary rounds.
• Assists patient/family with grieving process and end of life cases as appropriate.

Minimum Qualifications

• BSN or BS/BA degree in Nursing required.
• Current RN Pennsylvania state license required.
• Current BLS required. ACLS required or obtained by completion of orientation if applicable to area of practice.
• Previous acute care experience preferred.
