Qualifications

• Uses the nursing process to plan and deliver patient care.
• Performs ongoing assessment, diagnoses, goals, interventions, and evaluates outcomes.
• Documents patient care in records per health system standards.
• Provides teaching on Pain Management, Nutrition, Skin Integrity, Fall prevention, Diabetes, and Discharge instructions.
• Demonstrates commitment to patient safety, confidentiality, and ethical practice.

Responsibilities

• Applies nursing process to assess and address patient needs.
• Maintains comprehensive documentation in patient/unit records.
• Delivers patient and family education on health topics and discharge planning.
• Collaborates with interdisciplinary team to optimize patient outcomes.
