Job summary

Cape Cod Post Acute is seeking a skilled RN to deliver patient-centered care, monitor resident conditions, and implement care plans in our 135-bed Sub-Acute and Long-Term Care facility in Brewster, MA.

We offer full-time, part-time, and PRN roles with daily pay, shift differentials, and a $12,000 sign-on bonus for FT. An active MA RN license and BLS are required to join our compassionate team.

Qualifications

• Must hold an active MA RN license.
• Valid BLS (CPR) certification required.
• Ability to manage multiple residents in a fast-paced setting.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide resident care per physician's plan and written plan of care.
• Follow care plans developed by Nurse Manager.
• Administer medications and treatments per POC.
• Monitor residents, report changes to clinical team.
• Maintain accurate documentation per orders and policies.
• Collaborate with interdisciplinary teams for high-quality care.
• Ensure compliance with infection control and safety standards.
