Job summary

SolutionHealth in Nashua, NH is hiring a Registered Nurse for immediate care and ambulatory support. The role involves rooming patients, triage, vitals, and coordinating care with a multidisciplinary team across in-person and telehealth modalities.

You will administer medications, perform tests, and educate patients on managing health and wellness as part of a collaborative care plan. Prior healthcare experience and NH RN licensure are required.

Qualifications

• NH Registered Nursing licensure is required.
• Current BLS/CPR certification.
• Prior work experience in a healthcare setting is required.
• Epic experience is a plus.

Responsibilities

• Carry out the nursing process in person, by telephone and via telehealth to assess and plan patient care.
• Collaborate with Physicians, Physician Assistants and other health professionals to design and implement a care plan.
• Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; collect and handle specimens as allowed by state law.
• Educate patients on self-management, medications, nutrition, health and wellness per the care plan.
