Job summary

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is seeking a registered nurse to assist providers, room patients, take vitals, triage, call in scripts, and support immediate care services.

You must hold NH RN licensure, current BLS/CPR, and have prior healthcare experience; Epic experience is a plus. This role offers flexible day shifts and comprehensive benefits.

Qualifications

• NH Registered Nursing licensure is required.
• Current BLS/CPR training is required.
• Prior experience in a healthcare setting is required.
• Epic experience is a plus.

Responsibilities

• Carry out the nursing process in person, by telephone, and via telehealth modalities.
• Assess and identify patient needs, plan to achieve outcomes, intervene and evaluate progress.
• Collaborate to design and implement a care plan with the interdisciplinary team.
• Administer medications, treatments, vaccines, and perform point-of-care testing as allowed.
• Educate patients on self-management, medications, nutrition, and wellness per the care plan.
