Qualifications

• Current NH RN licensure is required.
• Certifications: BLS/CPR training.
• Prior work experience in a healthcare setting is required.
• Epic experience is a plus.

Responsibilities

• Carry out nursing process in person, by telephone, and via telehealth.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress.
• Collaborate to design and implement a plan of care with the interdisciplinary team.
• Administer medications, treatments, vaccines, and perform point-of-care testing.
• Educate patients on self-management and plan of care.
