Special Touch Home Care is seeking an experienced RN Field Supervisor to perform initial evaluations and ongoing reassessments for home care clients across Brooklyn. You will work independently, building meaningful relationships with clients while overseeing field staff and documenting care efficiently.

This per-visit position offers flexible weekday hours and requires travel throughout Brooklyn. The role emphasizes compassionate patient care, professional supervision, and adherence to home care

Qualifications

• Current and valid NYS RN License.
• Excellent written and verbal communication skills.
• Minimum of 1 year of RN home care field experience.
• Experience with UAS assessment/documentation is a plus.

Responsibilities

• Conduct initial evaluations and reassessments of home care clients in the field.
• Work independently while building relationships with clients.
• Provide field-based oversight and ensure caregiver performance.
• Handle internal case management and related documentation.
