Summary

Organizes, coordinates, and directs the ongoing provision of services to the patients assigned to them within the agency. Ensures the delivery of quality patient care and, when appropriate, provides supervision during all service hours, including supervision of LPNs and CNAs as required. Initiates and maintains communication with patients, families, physicians, other home health/hospice team members, and agencies to promote positive patient outcomes and appropriate use of healthcare resources.

Performs initial and follow-up comprehensive assessments, drug regimen reviews, and reconciliation. Collaborates with the patient, family, physician, and team members to develop and update the plan of care. Documents into the EMR at the point of care to ensure up-to-date patient records. Completes physician summaries at SOC, Recert, Transfer, ROC, and discharge, and documents communication with the team. Maintains an accurate visit schedule and participates in the agency's Nurse on-call schedule, weekly case conferences, and QAPI program. Demonstrates familiarity with agency policies and regulatory rules. May be assigned other duties as appropriate.

Job Qualifications

• Graduate of an accredited school of nursing
• Bachelor's and/or Master's degree preferred
• Current RN Missouri license in good standing
• Minimum two years of varied nursing experience, preferably including one year in home health/hospice
• Strong administrative, communication, and documentation skills
• Ability to assess and respond to patient and family needs in varied settings
• Skilled in nursing practice and critical thinking
• Ability to cope with emotional stress and diverse lifestyles
• Proficient in documentation and writing
• Ability to lift, transfer, or move items or patients weighing 10-200+ lbs with proper techniques and assistance when needed
• Ability to drive in all weather conditions
• Manual dexterity and skillful use of hands with patients and equipment

Pay range: $29.39 - $48.20 per hour
