Job summary

Boost Home Healthcare - Southeast Michigan is seeking a Field Registered Nurse to provide skilled nursing care in patients’ homes on an intermittent basis. You will work under physician orders and the Director of Patient Care Services to support recovery and wellness in the patient’s residence.

The role emphasizes compassionate, patient-centered care with flexible scheduling, field support, and ongoing development opportunities. Current Michigan RN licensure and BLS certification are required.

Qualifications

A passion to serve and help others live their best lives possible.
Graduate of an accredited school of professional nursing.
Currently licensed as a registered nurse in the state of the job posting.
Minimum of one (1) year of nursing experience in a home health setting.
Must have current BLS Certification.
Access to reliable transportation.

Responsibilities

Make the initial evaluation visit and regularly reevaluates the patient's nursing needs.
Meet Daily Productivity Standards.
Initiate the plan of care and necessary revisions.
Provide those services requiring substantial specialized nursing skills.
Initiate appropriate preventative and rehabilitative nursing procedures.
Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
