Job summary

Longevity Health Plan is seeking an RN Field Coordinator to blend direct patient care with administrative coordination in long-term care settings. You will travel to nursing homes to support Advanced Practice Providers, oversee transitions of care, and educate facility staff on our clinical model.

The role requires 2+ years as a Registered Nurse, strong EMR skills, and ability to travel. Join a national provider of I-SNP services and contribute to high-quality, patient-centered care.

Qualifications

• Minimum 2 years’ experience as a Registered Nurse.
• Excellent verbal and written communication skills.
• Excellent computer skills, EMR/clinical Platform/MS Office Products.
• Able to travel.
• Knowledge of long-term care and managed care coordination.

Responsibilities

• Conduct in-person visits at multiple nursing homes to assist APNs with clinical and administrative tasks.
• Prescreen new enrollees and identify continuity of care needs in facility EMRs.
• Oversee member appointments and ensure timely communication with providers and staff.
• Support transitions of care and assist with prior authorizations and access to services outside the facility.
• Educate facility staff on Longevity Health Plan model of care and lead orientation sessions.
