Job summary

Longevity Health is seeking an experienced RN Field Coordinator to blend hands-on patient care with administrative coordination in long-term care settings. The RN Field Coordinator travels to nursing homes to support Advanced Practice Providers and educate facility staff on our clinical model.

Ideal candidates have 2+ years as an RN, strong communication skills, and proficiency with EMR systems and MS Office. A valid RN license and driver’s license are required, with willingness to travel.

Qualifications

• Licensed RN in good standing.
• Valid state driver’s license with a good driving record.
• Auto insurance coverage as required by your state of residence.
• Associate degree in nursing required; BSN preferred.
• Minimum 2 years’ experience as a Registered Nurse.

Responsibilities

• Conduct in-person nursing home visits to assist Advanced Practice Providers with clinical and administrative tasks.
• Use facility EMRs to prescreen new enrollees and identify continuity of care needs.
• Collaborate with clinical and operations teams to oversee member appointments and ensure timely communication.
• Support transitions of care and help with prior authorization requests.
• Educate facility staff on Longevity Health Plan model of care.
• Lead orientation sessions for new and existing nursing homes to educate staff.
• Help initiate telehealth services in newly onboarded facilities, including staff education.
• Maintain proficiency in the Longevity Health Plan EMR and assist with data entry.
