Job summary

Longevity Health Plan is seeking an RN Field Coordinator to support the Institutional Special Needs Plan (I-SNP) through hands-on patient care and administrative coordination in long-term care settings.

The role involves traveling to nursing homes to assist Advanced Practice Providers with clinical tasks, coordinating transitions of care, and educating facility staff on the Longevity Health Plan model of care. A valid RN license is required and travel is expected.

Qualifications

• Licensed registered nurse in good standing.
• EMR/clinical platform familiarity is required.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct in-person visits at multiple nursing homes to assist advanced practice providers with clinical and administrative tasks.
• Prescreen new enrollees and identify continuity of care needs using facility EMRs.
• Collaborate with clinical and operations teams to oversee member appointments and care transitions.
• Assist with prior authorization requests and help members access services outside the facility.
• Communicate with patients, families, and physicians as delegated by nurse practitioners.
• Lead orientation sessions for staff on Longevity Health Plan model of care.
