Job summary

Longevity Health Plan seeks an RN Field Coordinator to blend hands-on patient care with administrative coordination in long‑term care settings. The role travels to nursing homes as assigned by the Market Director of Clinical Services to ensure smooth transitions of care and staff education on our model of care.

Required: RN license, valid driver’s license, and willingness to travel; 2+ years RN experience; EMR proficiency; strong communication and coordination skills. Equal opportunity employer.

Qualifications

• Licensed registered nurse in good standing.
• Valid state driver's license with a good driving record and proof of automobile insurance.
• Auto insurance coverage as required by your state of residence.
• Associate degree in nursing required; bachelor's degree preferred and RN licensed.
• Minimum 2 years' experience as a Registered Nurse.
• Ability to travel and coordinate care across facilities.
• Managed care coordination background/experience preferred.
• Excellent verbal and written communication skills.
• Experience with EMR/clinical platforms and data entry.

Responsibilities

• Conduct in-person visits at multiple nursing homes to assist practitioners with clinical and administrative tasks.
• Use facility EMRs to prescreen new enrollees and identify care needs.
• Coordinate with clinical and operations teams to manage member appointments and communication.
• Support transitions of care and assist with prior authorization requests.
• Educate facility staff on Longevity Health Plan model of care.
• Help initiate telehealth services in onboarding facilities.
• Maintain proficiency in Longevity EMR and assist with data entry.
• Participate in clinical meetings and hospitalization reviews.
• Perform other duties as assigned by the MDCS.
