Job summary

Lightways Hospice and Serious Illness Care is seeking a Field CNA to provide compassionate care in patients' homes across DuPage County and surrounding areas. The role supports the interdisciplinary team by assisting with activities of daily living and documenting patient observations under RN supervision.

The ideal candidate holds a current Illinois CNA certification, a high school diploma or GED, and a valid driver’s license with reliable, insured travel.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

Field CNA (DuPage County & Surrounding Territory)

The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The field CNA is responsible for observing the patient, reporting these observations and documenting observations and care performed.

Key Responsibilities

• Provide compassionate care, emotional support, and companionship to patients and families
• Assist with personal care, mobility, and daily living activities

• Position requires outside travel- Primarily DuPage County- will include surrounding area based on census

Education/Experience

• High school diploma or GED.
• Meets the training requirements of Illinois.
• Current CNA certification.
• Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.
