Job summary

Longevity Health is seeking an RN Field Coordinator to support transitions of care within nursing homes as part of the I-SNP program. This role blends hands-on patient care with administrative coordination, traveling to facilities as assigned by the Market Director of Clinical Services.

You will educate facility staff on our model of care, assist with prior authorizations, use EMRs, and collaborate across clinical and operations teams to ensure timely, coordinated care.

Qualifications

• Licensed RN in good standing.
• Associate degree in nursing required; bachelor’s degree preferred.
• Experience with EMR/clinical platforms is highly desirable.

Responsibilities

• Conduct in-person visits at nursing homes to support clinicians and staff.
• Prescreen enrollees and identify continuity of care needs using EMRs.
• Collaborate with clinical and operations teams to oversee appointments.
• Assist with prior authorizations and access to services outside facilities.
• Educate staff on Longevity Health Plan model of care.
