Humana Healthy Horizons in Kentucky is seeking a RN, Field Care Manager (Field Care Manager Nurse 2) to assess and evaluate prenatal and postpartum members' needs, guiding them toward and facilitating interactions with appropriate resources to promote their wellbeing. The role involves varied work assignments requiring interpretation and independent decision-making.

  • Employ strategies to manage members' physical, environmental, and psycho-social health issues.
  • Monitor patient progress through assessments and evaluations.
  • Create and maintain care plans for members.
  • Understand departmental, segment, and organizational strategies and objectives.
  • Make decisions regarding work methods, often in ambiguous situations, with minimal guidance.
  • Follow established guidelines and procedures.

Required Qualifications

  • Must reside in Kentucky.
  • Ability to travel within Kentucky (30-40 miles to members' residences).
  • Active, unrestricted RN license in Kentucky.
  • At least 2 years of maternal-child nursing experience (e.g., labor and delivery, neonatal ICU, postpartum/nursery).
  • Intermediate to advanced skills in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health, social services, and resources.
  • Ability to work a full-time, flexible schedule (minimum 40 hours).
  • Must participate in Humana’s TB screening program.
  • Possession of a valid driver’s license and vehicle insurance as per state requirements.

Preferred Qualifications

  • BSN degree.
  • 3-5 years in-home assessment and care coordination.
  • Experience in health promotion, coaching, and wellness.
  • Managed care or Medicaid experience.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.

Additional Information

  • Workstyle & Travel: Up to 10% travel for home visits, training, or meetings, including occasional visits to Humana’s Louisville, KY office.
  • Work hours: Monday-Friday, full-time, with flexible scheduling.

Work from Home Internet Requirements

  • Minimum download speed of 25 Mbps and upload speed of 10 Mbps recommended.
  • Wireless, wired cable, or DSL connections preferred; satellite or cellular only with approval.
  • Dedicated workspace required to protect PHI/HIPAA information.

Interview Process

Use of HireVue technology for interviews, allowing flexible scheduling.

Travel: Occasional travel to Humana offices or member homes.

Hours & Compensation: 40 hours/week; $71,100 - $97,800 annually, with potential bonuses based on performance.
