Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity member's needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for the care and well-being of pregnant and postpartum members. The RN, Field Care Manager Nurse 2 (Maternal-Health) will have varied work assignments requiring interpretation and independent decision-making.

Earn a $5,000 hiring bonus! $2,000 paid after 6 months and $3,000 after 1 year of employment, contingent on employment through those dates.
  • Conduct health risk assessments across the care continuum for members.
  • Create individualized care plans and participate in interdisciplinary team meetings.
  • Use various strategies to manage physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Monitor members' progress towards health outcomes through ongoing assessments.
  • Understand organizational strategies and operational objectives, including their linkages.
  • Make autonomous decisions regarding work methods, occasionally in ambiguous situations, with minimal guidance.
  • Collaborate with internal and external providers to meet member needs.
  • Ensure compliance with contractual and regulatory standards.
  • Follow established guidelines and procedures.

Required Qualifications

  • Reside in the central to northern Roanoke/Alleghany Region of Virginia, including Giles, Montgomery, Roanoke, Salem, Bedford, Craig, Botetourt, Covington, Alleghany, Rockbridge, Buena Vista, Lexington, Bath, and Highland.
  • Active RN license in Virginia without disciplinary action.
  • At least 2 years of healthcare and/or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
  • Proficient in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills, with the ability to build rapport quickly.
  • Ability to work independently within the role's scope.
  • Experience with electronic medical records and web navigation skills.
  • This role involves patient interaction and TB screening; successful candidates will be screened accordingly.
  • Must have a valid driver’s license, personal vehicle insurance, and be willing to travel regionally.

Preferred Qualifications

  • Bachelor’s of Science in Nursing (BSN).
  • 3-5 years of in-home assessment and care coordination experience.
  • Lactation certification, preferably as a lactation consultant.
  • Experience in health promotion, coaching, and wellness.
  • Managed care experience.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual abilities in English and another language such as Spanish, Arabic, Vietnamese, Amharic, or Urdu.

Additional Information

  • This is a remote position requiring travel to meet members, families, and providers, with occasional onsite meetings in Glen Allen, VA.
  • Work hours are Monday to Friday, 8:00 am – 5:00 pm EST.
  • Travel is estimated at 50-75% regionally.
  • Applicants must complete a language proficiency assessment if speaking with members in a language other than English.

Internet Requirements

  • Minimum internet speed of 25 Mbps download and 10 Mbps upload.
  • Wireless, wired cable, or DSL connections preferred; satellite or cellular only with approval.
  • Work from a dedicated, interruption-free space to protect member information.

Pay Range

$71,100 - $97,800 annually, commensurate with experience and location. Eligible for performance-based bonuses.
