RN, Field Care Manager Nurse (Maternal-Child Health)

Humana Healthy Horizons in Kentucky is seeking a RN, Field Care Manager (Field Care Manager Nurse 2) who will assess and evaluate prenatal & postpartum members' needs to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for their care and wellbeing. The RN, Field Care Manager's work assignments are varied and often require interpretation and independent decision-making.

  • Employs strategies and techniques to manage physical, environmental, and psycho-social health issues.
  • Monitors patient progress through assessments and evaluations.
  • Creates and maintains members’ care plans.
  • Understands department, segment, and organizational strategies and objectives.
  • Makes decisions regarding work methods, sometimes in ambiguous situations, with minimal guidance.
  • Follows established guidelines and procedures.

Required Qualifications

  • Must reside in Kentucky or a contiguous county in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia.
  • Ability to travel within Kentucky (30-40 miles).
  • Active, unrestricted RN license in Kentucky.
  • Two years of maternal-child nursing experience (labor and delivery, neonatal ICU, postpartum/nursery).
  • Intermediate to advanced skills in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social service agencies.
  • Full-time (40 hours minimum) flexible schedule.
  • Must participate in Humana’s TB screening program.
  • Valid driver’s license and vehicle insurance in accordance with state requirements.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • 3-5 years in-home assessment and care coordination.
  • Experience with health promotion, coaching, and wellness.
  • Managed care or Medicaid experience.
  • Certification in Case Management (CCM).
  • Knowledge of Motivational Interviewing.
Additional Information
  • Workstyle & Travel: Up to 10% travel, including home visits and meetings at Humana’s Louisville, KY location.
  • Work hours: Monday to Friday, full-time, flexible schedule.

  • Minimum download speed of 25 Mbps and upload speed of 10 Mbps.
  • Wireless, wired cable, or DSL connection preferred.
  • Satellite, cellular, or microwave must be approved.
  • Dedicated workspace to protect member information.
Interview Process

Occasional travel to Humana’s offices may be required.

Compensation and Benefits

Salary: $71,100 - $97,800 annually, with potential bonuses based on performance.
