Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity members' needs to promote wellness by guiding members and families toward appropriate resources, especially for pregnant and postpartum members. The role involves varied assignments requiring interpretation and independent decision-making.

  1. Conduct health risk assessments across the care continuum.
  2. Create individualized care plans and participate in interdisciplinary team meetings.
  3. Use various strategies to manage physical, environmental, and psycho-social health issues.
  4. Identify and resolve barriers to effective care.
  5. Monitor members' progress through assessments and evaluations.
  6. Understand organizational strategies and objectives, linking them to related areas.
  7. Make decisions regarding work methods, often in ambiguous situations, with minimal guidance.
  8. Collaborate with internal and external providers to meet member needs.
  9. Ensure compliance with contractual and regulatory standards.
  10. Follow established guidelines and procedures.

Required Qualifications

  • Residency in the central to northern Roanoke/Alleghany Region, Virginia: counties of Giles, Montgomery, Roanoke, Salem, Bedford, Craig, Botetourt, Covington, Alleghany, Rockbridge, Buena Vista, Lexington, Bath, Highland.
  • Active RN license in Virginia without disciplinary action.
  • At least 2 years of healthcare or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women's care, labor and delivery, NICU, or clinical triage.
  • Proficiency with Microsoft Office Suite.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills.
  • Ability to work independently within scope.
  • Experience with electronic medical records and web navigation.
  • Must pass Tuberculosis screening.
  • Valid driver's license and personal vehicle insurance for travel.

Preferred Qualifications

  • Bachelor's Degree in Nursing (BSN).
  • 3-5 years of in-home assessment and care coordination.
  • Certified lactation consultant or lactation experience.
  • Experience in health promotion, coaching, wellness, or managed care.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual in English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.

Additional Information

  • Workstyle: Remote position with regional travel required.
  • Travel: 50-75% to meet members, families, and providers; occasional onsite in Glen Allen, VA.
  • Work hours: Monday-Friday, 8:00am-5:00pm EST.
  • Language assessment: Non-English speakers must complete an ILR language proficiency test.

Internet Requirements

  • Minimum download speed: 25 Mbps; upload speed: 10 Mbps.
  • Wireless, wired, or DSL connection preferred; satellite or cellular only with approval.
  • Dedicated workspace to protect member information.

Compensation: $71,100 - $97,800 annually, with potential bonuses based on performance.
