Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity members' needs to achieve and maintain optimal wellness. The role involves guiding members and families toward appropriate resources for the care of pregnant and postpartum members. The assignments are varied and often require independent judgment.
- Responsible for health risk assessments across the continuum of care.
- Creates individualized care plans and participates in interdisciplinary team meetings.
- Employs various strategies to manage physical, environmental, and psycho-social health issues.
- Identifies and resolves barriers to effective care.
- Monitors members' progress towards health outcomes through ongoing assessments.
- Aligns with department and organizational strategies and objectives.
- Makes independent decisions regarding work methods, with guidance as needed.
- Collaborates with providers and internal departments to meet members' needs.
- Ensures compliance with contractual and regulatory requirements.
- Adheres to established guidelines and procedures.

Required Qualifications
- Must reside in the central to northern Roanoke/Alleghany Region in Virginia, including Giles, Montgomery, Roanoke, Salem, Bedford, Craig, Botetourt, Covington, Alleghany, Rockbridge, Buena Vista, Lexington, Bath, and Highland.
- Active RN license in Virginia without disciplinary action.
- Two years of healthcare and/or case management experience.
- At least one year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Intermediate to advanced skills in Microsoft Office applications.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills, with the ability to build rapport quickly.
- Ability to work independently within the role and scope.
- Experience with electronic medical records and other information systems.
- Ability to travel regionally for face-to-face assessments.
- Must pass TB screening and have a valid driver’s license with appropriate insurance.
Preferred Qualifications
- Bachelor’s of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination experience.
- Certified lactation consultant or lactation experience.
- Experience in health promotion, coaching, and wellness.
- Previous managed care experience.
- Certification in Case Management (CCM).
- Motivational Interviewing certification or knowledge.
- Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages, with proficiency in speaking, reading, and writing.
Additional Information
- This is a remote position requiring travel.
- Travel estimated at 50-75% to meet members, families, and providers; occasional onsite meetings in Glen Allen, VA.
- Work hours: Monday–Friday, 8:00 AM – 5:00 PM EST.
- Language proficiency assessments may be required for non-English speakers.
Technical Requirements
- Home internet must meet minimum speeds: 25 Mbps download and 10 Mbps upload.
- Work from a dedicated, interruption-free space to protect PHI/HIPAA information.

Compensation and Benefits
Starting salary ranges from $71,100 to $97,800 annually, with potential bonuses based on performance. Benefits include health coverage, 401(k), paid time off, disability, life insurance, and more.
