Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. In this role, you’ll provide clinical triage, care coordination, patient education, and post‑discharge follow‑up to improve health outcomes and patient experience.

The RN III position involves advanced training and leadership responsibilities, including program administration and staffing development. Evening shifts support a stable outpatient schedule and work‑life balance.

Qualifications

• BSN strongly preferred or graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.
• Minimum of 3 years of clinical experience in ambulatory care or specialty, and/or demonstrated competency in IHA’s RN II position.

Responsibilities

• Proficient to work within scope of nursing certification as applicable (e.g., lactation consulting, advanced wound ostomy care, care/case management).
• Serves as a nurse leader by assisting management with hiring, motivating and engaging clinical staff.
• Identifies and targets at‑risk patients to improve outcomes.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post‑discharge follow‑up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
