Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. The RN III role emphasizes leadership, care coordination, patient education, and support across transitions of care in an ambulatory setting.

The part-time, evening-shift position offers opportunities to influence patient outcomes while collaborating with providers, care managers, pharmacists, and behavioral health specialists.

Qualifications

• RN license to practice in Michigan (unrestricted) and CPR certification.
• Minimum 3 years of ambulatory care or specialty nursing experience.
• BSN strongly preferred or graduate with advanced certification/training relevant to specialty (e.g., AAACN, IBCLC).

Responsibilities

• Proficient to work within the scope of nursing certification, including lactation, ostomy, diabetes educator or asthma educator credentials.
• Assist management with hiring, motivating and engaging clinical staff as a nurse leader.
• Identify at‑risk patients requiring extensive care to improve outcomes.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post‑discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and behavioral health specialists.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.

Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Registered Nurse (RN) to join our outpatient care team. In this role, you'll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience. The RN III works in a capacity that requires advanced training, education, certification and/or leadership. Additionally, RN III’s may have program administration and staffing development responsibilities.

What You'll Do

What We're Looking For

• EDUCATION: BSN strongly preferred or Graduate of an accredited Registered Nurse program. Advanced certification/training relevant to specialty (e.g., AAACN, IBCLC) or graduate degree preferred.
• CREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan. Valid CPR certification.
• MINIMUM EXPERIENCE: 3 years of clinical experience in a ambulatory care or specialty, and/or demonstrated competency in IHA’s RN II position.
