Responsibilities

• Completes comprehensive assessments to identify intervention opportunities.
• Plans safe, efficient discharge and transition plans considering clinical and social factors.
• Coordinates care progression, authorizations, and communication with payers and teams.
• Facilitates tests, reviews, and documentation to ensure medical necessity and efficient care.
