Job summary

MLK Community Healthcare is seeking an RN Care Manager for Emergency Room to support physicians and the interdisciplinary team in facilitating patient care, improving outcomes, and managing costs. The role emphasizes utilization management, care progression, and transitions across the hospitalization continuum.

Qualifications

• Minimum 1–3 years of hospital or related experience.
• Internals with at least 18 months acute care case management experience will be considered in lieu of nursing clinical experience.
• Bilingual Spanish preferred; basic computer skills required.

Responsibilities

• Coordinate precertification and payor authorization processes.
• Collaborate with physicians and interdisciplinary team for care progression and discharge planning.
• Assess patient needs, plan care, and facilitate transitions across the continuum.
• Communicate with payors, physicians, and patients to ensure timely and appropriate care.
