• Serves as primary coordinator of disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Coordinates discharge planning needs with the healthcare team.
• Demonstrates ability to assess holistic needs of patients and provide individualized care.
• Delegates care to nursing staff as appropriate to optimize outcomes.
• Participates in daily interdisciplinary rounds and contributes input for patient care.
