Qualifications

• BSN degree from an NLNAC/CCNE accredited program.
• 5+ years of total nursing experience with recent discharge planning
• Experience applying InterQual/Milliman guidelines
• Strong coordination with care teams and payers

Responsibilities

• Provides discharge planning for inpatients, same-day surgery and ED patients.
• Develops processes to improve efficiency when discharge planning is high demand.
• Predicts discharge needs to avoid delays and excessive LOS.
• Coordinates transfers and obtains insurer authorizations as needed.
• Documents care plans and communicates with patients, families and teams.
• Works with DoD/DoN/DoD-W policies and DoD/MCSC requirements for post-discharge care.
