Job summary

Wellpath / Correct Care Solutions in Eureka, CA seeks an RN Discharge Planner to coordinate daily discharge planning and care management in a correctional healthcare setting. You will oversee assessments, ensure regulatory compliance, and collaborate with managed care partners to facilitate safe transitions.

Ideal applicants hold an unrestricted RN license in California, a BSN, and 2+ years in discharge coordination, with CCM preferred.

Qualifications

• Unrestricted, current RN license in state of employment.
• BSN; CCM preferred.
• Two years clinical experience with discharge coordination.
• Experience in correctional/detention facilities.
• CalAIM familiarity preferred.

Responsibilities

• Coordinate clinical reviews and discharge planning activities.
• Review documentation, referrals, and pre-certification requests for compliance.
• Collaborate with medical directors, providers, site personnel, and care management teams.
• Monitor utilization data, coding accuracy, and cost management.
• Provide clinical oversight of reentry care plans and coordinate with managed care plans.
