Job summary

Glastonbury Center for Health & Rehabilitation in Glastonbury, CT, is seeking an RN Discharge Planner to ensure our short-term, skilled nursing patients have medically stable, safe discharge plans to promote success in the community.

You will collaborate with the treatment team to create care plans, review documentation, and coordinate coverage to limit readmissions, while advocating for residents' rights and preferences.

Qualifications

  • Valid state RN license preferred.
  • Interest in the nursing needs of the aged and chronically ill.
  • Experience in skilled nursing facilities, clinical reimbursement, concurrent review and/or utilization management is desirable.
  • Experience/knowledge of Medicare guidelines, InterQual and/or McKesson.
  • Excellent communication and interpersonal abilities; proficient with Microsoft Office 365.

What You'll Do:

What You'll Bring:
