This is an Hourly position. Pay range for this position is $33.91 - $51.04/hour. Actual pay is based on years of licensure. This position also includes a stackable $3.25/hr weekend differential.
POSITION SUMMARY: The Digital Hospital Admission Nurse Specialist will identify and screen potential patients who are appropriate for admission into Digital Hospital. Once identified, the Nurse Acquisition Specialist will present the Digital Hospital program to the patient and obtain consent. The Digital Hospital Admission Nurse Specialist is responsible for identifying, assessing, planning, coordinating, implementing, monitoring, and evaluating options and services across the continuum of care for individuals in the Digital Hospital. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level, optimizing clinical outcomes and satisfaction with services, as determined by individualized care plans. The Nurse Admission Specialist assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The Nurse Acquisition Specialist assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans. Additionally, demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Flexible work schedule required, including weekend and holiday rotations.
