Centers Dialysis Care is seeking Registered Dialysis Nurses (RN) for our home hemodialysis program in a skilled nursing facility located in Bronx, NY.

Duties

1. Assess patient status and implement care plans collaboratively with the interdisciplinary team.
2. Coordinate care and educate patients.
3. Administer medications and treatments as directed.
4. Monitor patient responses and document accordingly.
5. Mobilize resources in complex cases to enhance patient participation.
6. Use assessment and diagnostic information to anticipate healthcare needs.
7. Ensure effective handoff communication and reporting.
8. Adjust staffing and develop care plans proactively.
9. Provide individualized comfort measures using evidence-based practices.
10. Participate in performance improvement activities.
11. Perform other related duties as required.

Requirements

• Active NY RN License
• Dialysis experience required
• Nx Stage machine experience a plus
• CPR Certification

Rate: $45.00 - $55.00 per hour
