Qualifications

• Bachelor's Degree in Nursing required.

• Experience as a Diabetes Educator preferred.
• CBDCE CDCES certification within 2 years required.
• BC-ADM certification within 2 years required.
• RN license in state of employment or compact state required.
• CPR/BLS certification required.

Responsibilities

• Provide education to individuals and groups on a wide variety of topics with emphasis on diabetes education.
• Systematically collect and assess data related to patient health status to develop, execute, and evaluate the plan of care.
• Establish goals and strategies for meeting care coordination needs across settings with patient, family, and/or care providers.
