Job summary

Froedtert Health in Manitowoc is seeking a part-time Diabetes Educator to perform in-depth diabetes assessments, develop plans of care, and provide outpatient education to patients and the community.

The role requires Wisconsin RN licensure (or RD/CDE credentials), strong teaching skills, and the ability to work independently as part of a care team. BLS certification is expected, with opportunities for professional development.

Qualifications

• Graduate from an accredited school of nursing with current license to practice in the State of Wisconsin required.
• Registered Dietician also acceptable.
• Certified Diabetic Educator (CDE) certification preferred.

Responsibilities

• Performs in-depth assessments and formulation of individual plans of care for people with diabetes.
• Acts as a resource to professional staff and the community, providing outpatient diabetes education.
• Participates in hospital and community in-services and support groups.
• Investigates grant funding for projects associated with diabetes and chronic conditions associated with diabetes.
