Job summary

The RN Patient Educator – Diabetes Care and Education Specialist at Cleveland Clinic's Main Campus coordinates the diabetes education program and provides educational resources to patients and caregivers.

This full-time, day-shift role located in Cleveland, OH collaborates with Endocrinology providers to design and administer nutrition and diabetes care, performs comprehensive patient assessments, and develops individualized care plans aligned with national standards.

Qualifications

• Education: Bachelor's degree or higher in nursing, dietetics, or a related field.
• Experience: One year of experience as a Registered Dietitian or Registered Nurse.
• Licenses & Certifications: Current license to practice dietetics or nursing in the State of Ohio; CDCES upon hire or within two years; Dietitians must have completed a dietetic internship and have active RD (RN) or Nutritionist (RDN) certification.

Responsibilities

• Assist with the overall coordination of the diabetes education program.
• Provide educational resources to physicians, caregivers, and patients.
• Facilitate the organizational change process to promote and implement services.
• Provide direct patient care using medical nutrition therapy or diabetes self-management education.
• Collaborate with Endocrinology providers to design and administer nutrition and diabetes care.
• Complete comprehensive assessments for each patient, including emotional and behavioral health.
• Develop individualized care plans with safe and effective interventions.
• Utilize national standards and guidelines for diabetes and obesity care in practice.
• Deliver specialized interventions and comprehensive counseling and education services.
• Implement care plans, communicate plans, and maintain documentation.
