Job summary

Nevada Health Centers in Las Vegas, NV seeks an RN Diabetes Educator & Case Manager to provide comprehensive diabetes self-management education and support within a team-based care model. This full-time role collaborates with providers, pharmacists, and case managers to improve patient outcomes and access to care.

Qualifications

• Graduate of an accredited nursing school.
• Licensure as a registered nurse in the state of Nevada.
• Current CPR or BLS certification.
• Active Nevada Driver's License.
• A minimum of three years of experience in a clinical practice.

Responsibilities

• Direct patient education in-person and virtually.
• Provide medication and insulin teaching to patients.
• Provide glucose monitoring support to patients.
• Provide lifestyle counseling to promote optimal self-management with patients with Diabetes and other chronic diseases.
• Provide and participate in care coordination with primary care and specialists.
• Coordinate and conduct home visits as needed.
• Interpret data and trends from multiple data sources including remote patient monitoring, using appropriate analytical and critical thinking skills.
• Provide timely and accurate documentation, tracking and reporting of diabetic clinical outcomes.
• Provides timely responses to inquiries from health plans, providers and other members of the patient care team.
