UnitedHealth Group is seeking an RN Diabetes Educator in Little Rock, AR to deliver comprehensive diabetes education and self-management support. You will integrate clinical knowledge with patient education, deliver care via telehealth and in-person visits, and participate in quality improvement initiatives with the Level2 care team.

The ideal candidate has RN licensure with a compact license and 1+ year outpatient diabetes education experience; a CDES certification is required.

• 1+ years of outpatient diabetes self-management education and support experience required.
• Certified Diabetes Care and Education Specialist required.

About This Role

The RN Diabetes Educator delivers comprehensive services that integrate clinical and self-management aspects of type 2 diabetes. This role is ideal for those who are passionate about improving the quality of care and outcomes for individuals living with diabetes.

• Pay: $60,200 - $107,400/year
• Location: Little Rock, AR
• Schedule: Full Time, Day shift

What You'll Do

• Integrate skills and knowledge of pathophysiology, epidemiology, clinical management, and self-management of diabetes into clinical practice.
• Advocate for and communicate about improved quality of care and outcomes for those living with type 2 diabetes.
• Actively participate in the quality improvement process and adapt practice/process based on quality findings.
• Partner with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes.
• Apply the AADE7 self-care behaviors to educate on and initiate behavior change.
• Engage in lifelong learning and serve as a role model of professionalism.
• Provide quality diabetes self-management education via telehealth visits based on assessed needs.
• Follow the National Standards of Diabetes Self-Management and Support Guidelines.
• Complete comprehensive assessments for each patient and develop individualized care plans.
• Document all individual contacts/visits in the Electronic Health Record and outcomes database.
• Collaborate with other members of the Level2 care team in developing person-centered diabetes plans.
• Evaluate and analyze continuous glucose monitor data to guide diabetes education and support.
• Contribute to the achievement of established department goals and objectives.

Requirements

Education:

• Current and unrestricted RN compact license required.

Experience:

• 1+ years of outpatient diabetes self-management education and support experience required.

Licenses & Certifications:

• Certified Diabetes Care and Education Specialist required.
