Job summary

Qualifications

• RN license with potential for multi-state license within 30 days of hire
• Minimum 1 year nursing experience; hospice or hospital experience preferred
• Active BLS for healthcare professionals (AHA or Red Cross)
• Experience with EMR/EHR systems
• English communication skills; Spanish a plus

Responsibilities

• Triages phone calls and ensures callers' needs are met with excellent customer service
• Provides and manages direct care to patients and families, incorporating psychosocial, spiritual, cultural, physical and biological components
• Handles clinical inquiries related to Chapters Health System’s services and promotes their benefits
• Gathers information and documents patient data for EMR
• Documents in EMR and coordinates in-person visits when needed
• Supports patients and families to achieve desired comfort levels
• Collaborates with resources to resolve complex situations
• Performs other duties as assigned
