Job summary

Chapters Health in Florida is seeking an experienced RN for Day Triage to provide telephonic and ad hoc support to patients and families, establishing positive relationships with team members and referral resources.

The role requires an active RN license, at least one year of nursing experience (hospice/hospital preferred), BLS certification, EMR/EHR experience, strong communication, and the ability to work in a fast-paced, team-oriented environment while delivering compassionate care.

Qualifications

Current RN license (state or multi-state) and ability to obtain multi-state license within 30 days.
Minimum 1 year nursing experience; hospice/hospital experience preferred.
Active BLS for healthcare professionals.
Experience with EMR/EHR systems.
Ability to communicate in English; bi-lingual in Spanish preferred.

Responsibilities

Triages phone calls and ensures callers' needs are met with excellent service.
Provides and manages direct patient/family care with psychosocial elements and nursing interventions.
Manages documentation in EMR and coordinates referrals and follow-ups.
Assists patients and families to achieve comfort and access needed resources.
Triage RN issues escalate or coordinate onsite visits when needed.
