Job summary

Hackensack Meridian Health in North Bergen, NJ is seeking a Registered Nurse for a full-time day-shift role. You will provide direct patient care as part of an interdisciplinary team, collecting data and performing age- and disease-specific assessments to guide care decisions.

You will develop and implement care plans, communicate with patients and families, and document in real time to support optimal outcomes.

Qualifications

• BSN in progress and/or willing to acquire within three years of hire.
• Six (6) or more months of relevant nursing experience OR Nurse Residency Program.
• NJ State Professional Registered Nurse License; BLS within 60 days of start.

Responsibilities

• Collect comprehensive data pertinent to the patient's health or situation.
• Perform age and disease specific assessment/data collection independently.
• Recognize clinical and diagnostic status changes in patient status.
• Identify education needs based on age and disease.
• Develop a plan of care with strategies and interventions.
• Formulate patient/family centered goals/outcomes by age and disease.
• Communicate, coordinate, and implement the plan of care.
• Provide ongoing communication with patient/family and healthcare team.
• Implement age and disease specific care in an organized manner.
• Perform continuous Point-of-Care electronic documentation.
