Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required
• Active RN license required in state of practice
• Effective communication and teamwork in clinical settings

Responsibilities

• Interacts with co-workers, patients, and families to deliver patient-centered care
• Develops nursing plans of care with patients and designated care partners
• Utilizes information technology to communicate, monitor outcomes, and improve quality and safety of care
• Participates in hospital and practice committees focusing on safety and quality improvement
• Precepts and mentors appropriate staff in clinical settings
• Assists in discharge planning and post-discharge follow-up as needed
