Job summary

Traditional Home Health Care is seeking an RN for its Crisis Care Hospice Division. This per diem role offers flexible scheduling with 8-hour shifts (8AM–4PM, 4PM–12AM, 12AM–8AM). The RN will support hospice patients and families, perform triage, and provide compassionate end-of-life care in the home.

Ideal candidates have hospice experience and PA RN licensure, with after-hours availability and a commitment to teamwork in a patient-centered, community-based setting.

Qualifications

• Experience with hospice patients or hospice environment is a plus.
• RN candidate preferably with 1 year general nursing, with hospice/home health/medical/surgical/critical care experience.

Responsibilities

• Provide direct patient care and support to hospice patients and families.
• Conduct phone triage and occasional after-hours home visits for hospice patients.
• Collaborate with the interdisciplinary health care team to coordinate care and ensure comfort and dignity.
• Support families in managing practical details during end-of-life care.

The ideal candidate

RN in our Crisis Care Hospice Division is an aspiring individual looking to grow in an industry with the company.
The ideal candidate would work to uphold the quality of life for both the dying individual and the caregiver.
Our RN Hospice Nurses are involved in helping patients, and their families feel more comfortable about death and providing them with the emotional support they need.
Additionally, hospice nurses will do their best in assisting family members to manage any practical details that may be involved when caring for a dying loved one.
Able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and employees.

Experience

• Having experience with hospice patients or in the hospice environment is a plus.


• RN Candidate would preferably have (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience.


• RN to perform phone triage and prn home visits for our hospice patients during after hours


• Ability to work within an interdisciplinary setting.


• Understanding of Hospice philosophy, principles of death/dying


• Must have PA License
