Job summary

HCA Healthcare is seeking an RN Coordinator to facilitate patient transfers, placement, and movement across markets served. You will collect clinical information, match patient needs to bed capacity, and collaborate with referring facilities and hospital staff to ensure safe flow and throughput.

The role requires nursing education, clinical experience in acute care, and active RN licensure in applicable states.

Qualifications

• Graduate of an accredited nursing program required. Bachelor’s degree preferred.
• Minimum of 3 years related clinical experience in an acute care setting: ED, ICU, Critical Care highly preferred.
• RN License in the states where the HCA patient transfer center is located; Compact/multi-state license may be required if Transfer Center covers more than one state.

• Triage and prioritize patient information and communicate an accurate clinical presentation to accepting physicians, nurses, and hospital administrators.

Job description

The RN Coordinator uses acquired nursing knowledge and experience to facilitate the transfer, transport, and placement of patients for all HCA facilities within the markets served. This vital member of the team collects clinical information relative to a patient movement request and then facilitates the patient’s movement by matching the patient’s clinical needs to the facility with bed capacity and service capability; including collaboration for appropriate bed placement within the facility in accordance with the current transfer center processes. The RN Coordinator demonstrates initiative and problem‑solving skills while working with multiple and various departments within each hospital to foster relationships with referring facilities, physicians, and hospital staff. The position works closely with the leadership team at each facility to identify, trend, and report barriers to patient throughput and patient safety, for optimal clinical outcomes for the patient. This position will use customer service skills and professional communication standards to positively interact with peers, customers, and supervisors.

Primary Duties And Responsibilities

• Perform all transfer center functions, including centralized patient placement, in a timely, customer centered professional manner with the aid of a computer database consisting of facility service profiles, approved clinical protocols, established patient care protocols, and healthcare information.
• Coordinate transfer of emergent, urgent and elective patients to a destination facility from all potential referral sources to include but not limited to hospitals, clinics, post‑acute centers, and physician offices via a single call to initiate acceptance and expedite decision making relative to a potential transfer all while having an ongoing focus on quality and efficiency, providing follow up to all participants of the transfer.
• Ensure accurate and concurrent data collection and maintenance in both patient record and center workflow systems.
• Demonstrate the ability to process patient acuity utilizing critical thinking skills, clinical decision making, and acquired knowledge to assist in assessing situations and facilitate patient flow through the continuum of care. You will also triage and prioritize patient information and communicate an accurate clinical presentation of the patient for accepting physicians, nurses, and hospital administrators.

Education

• Graduate of an accredited nursing program required. Bachelor’s degree preferred.

Experience

Scheduling Requirements

• This position is a Full‑Time mixed shift position with a Weekend Rotation: 12 hour shift requirement unless otherwise noted. This role will require you to work some holidays.

Location

• This position is not remote and will is located at the Patient Logistics Center in Jacksonville FL. Qualified candidates must live within 60 miles of an HCA PLC in the market area associated with the position.
