Job summary

Prisma Health is seeking an experienced RN Case Manager to coordinate care across inpatient, ambulatory, and home settings. You will develop and monitor Plan of Care with patients and families, provide education via teach-back, and ensure smooth transitions across the continuum of care.

Responsibilities include home visits, collaboration with the care team, and documenting outcomes to improve clinical results and patient satisfaction.

Qualifications

• RN licensed and in good standing per NCSBN standards.
• Strong foundation in care management and patient education.
• Ability to collaborate with inpatient, ambulatory and home care teams.
