Qualifications

• RN with Minnesota license; BSN preferred.
• Minimum of 3 years clinical practice in acute care and 3 years in utilization review, discharge planning, or case management.
• Demonstrated independent nursing judgment and collaboration in care teams.
• Knowledge of quality improvement, utilization management, and healthcare payment methods.

Responsibilities

• Provide patient and family education to empower informed health care decisions.
• Coordinate services with physicians, health plan, and community resources.
• Identify interventions to meet member health goals and predict future needs.
• Communicate effectively with patients, families, and care team; document appropriately.
• Maintain confidentiality and compliant case management records.
• Report member outcomes to leadership and address high-cost cases as needed.
• Participate in committees and quality improvement initiatives.
