Prisma Health is seeking a registered nurse to coordinate an interdisciplinary plan of care for a designated patient population at our Columbia corporate facility. You will serve as a clinical resource, educator, and consultant to the team and community.

You will participate in direct and indirect patient care, collaborate with health professionals to develop and evaluate quality programs, and advance patient education. Day shifts, opportunities to impact care across departments.

Qualifications

• Education - Associate degree in Nursing; Bachelor's preferred.
• Experience - Three (3) years of nursing experience.

Responsabilités

• Coordinate an interdisciplinary plan of care for the specified patient population.
• Participate in direct and indirect patient care, rounds, and multidisciplinary care planning.
• Develop, implement, and evaluate quality programs for population needs in a cost-effective way.
• Develop, provide, and evaluate educational programs for patients, families, staff, and the community.
• Monitor documentation accuracy and maintain required reports and statistics.
• Monitor care quality and drive continuous process improvement.
• May participate in research when indicated.
• Perform other duties as assigned.
