Job summary

Brickyard Healthcare in Bloomington, MN is seeking a dedicated Registered Nurse (RN) to provide high-quality nursing care and support resident well-being. You will work with an interdisciplinary team to develop and implement individualized care plans.

You will collaborate with physicians and staff to ensure safe, effective discharge planning and continuous improvement in resident outcomes. A current MN RN license and CPR certification are required or obtained soon after hire.

Qualifications

• Licensed RN in state of practice required.
• CPR certification required or must be obtained within 90 days of hire.
• License in good standing with no abuse findings.

Responsibilities

• Deliver quality nursing care focused on outcomes using the nursing process.
• Identify needs and input for discharge planning and follow-up during stay.
• Collaborate with resident, family, and care team to develop an individualized plan of care.
• Implement the plan consistently and cost-effectively with resident-centered outcomes.
• Share information with physicians and other health professionals to adapt care plans.
• Maintain optimum resident care and comfort with current nursing practices.
