Job summary

Brickyard Healthcare is seeking a dedicated Registered Nurse to provide high-quality, resident-centered care. The RN will work as a collaborative team member to assess, plan, implement, and evaluate individualized care plans that prioritize patient comfort and outcomes.

The role emphasizes adherence to nursing processes, coordinated discharge planning, and close collaboration with physicians and other care professionals to ensure safe, effective care throughout the resident's stay.

Qualifications

• Currently licensed Registered Nurse in state of practice
• Must hold and maintain a current CPR certification
• License in good standing with the state; no finding for abuse/neglect/misappropriation

Responsibilities

• Ensure delivery of effective quality nursing care using the nursing process
• Identify needs and input for discharge planning and follow-up
• Collaborate with resident/family and interdisciplinary care team to develop plan of care
• Implement plan of care consistently and cost-effectively with focus on resident-centered outcomes
• Collaborate with physician and health care professionals by sharing care plan updates
• Deliver and maintain optimum resident care and comfort with current nursing practices
